Permissions to use photos

Yes, I give consent for Fasttrack-Languages to photograph my child for school purposes and/or at school events. No, I do not authorize Fasttrack-Languages to photograph my child for any event.

Accept Terms

Please sign the waiver below. The waiver and liability must be signed to have a valid registration. The Fasttrack-Languages center is well childproofed and the children are consistently well supervised. However, accidents do happen. The undersigned(s) assume(s) all risk of injury to the child associated with participation in any program including Summer Programs, Spring Programs, Reading, Writing, Math, Toddler Program, Gymnastics, Movement, Spanish, Italian, French and Mandarin and agree(s) to release, indemnify, defend and forever discharge Fasttrack-Languages and it's staff, employees, of and from all liability, claims, demands, damages, costs, expenses, actions and causes of action in respect of injury to the child, or by the child, howsoever caused, arising or to arise by reason of or during the child's participation in any of the classes of the school program.
